Supportive Housing Program |
|
Our supportive housing program provides personal care and
homemaking services to assist frail seniors to live
independently at Bracondale. Care is provided to seniors
65 years of age and over who are eligible for the services
following an assessment of needs.
Personal care includes assistance with dressing, grooming,
bathing and reminders about medication.
Homemaking
services involve light housekeeping, laundry, shopping and
preparation of light meals (breakfast and dinner only).
The
Supportive Housing Services are free, funded through the
Toronto Central LHIN (Local Health Integration Network).

Emergency Response: |
|
Is offered
24 hours a day. (Emergency pull cords are located
in the bathroom of each unit.) A daily safety
check is provided at noon, seven days a week for seniors
who sign up to use the service. Evening safety
checks before 9:00 p.m. are provided. |

meals (salad, entrée, dessert and coffee/tea) are offered in the
Bracondale dining room 7 days a week. |
